enrollment for changes to employee health and dental plans is
effective immediately through September 15, 2011
More information on the open enrollment
process follows the information on the Health Enhancement
Program. Even if you do not plan on making changes to your
current coverage it is vital that you read the information below
regarding the Health Enhancement Program.
Failure to follow the enrollment steps
below WILL RESULT IN INCREASED COSTS TO YOUR HEALTHCARE ($100
PER MONTH WILL COME OUT OF YOUR CHECK BEGINNING WITH THE OCTOBER
INFORMATION FOR ALL EMPLOYEES
Aside from the open enrollment period, ALL
employees must consider and elect to enter the Health
Enhancement Program to avoid incurring a $100 per month charge
beginning with the October 7, 2011 check. This
election should occur as soon as possible, but
no later than
September 15. If you
fail to elect to enroll in the Health Enhancement Program during
this period you will be charged an additional $100 per month in
premium costs and you will have an annual $350 per
individual (to a maximum of $1400 per family ) in-network
medical deductible. You will not be able to change your
enrollment status until the next open enrollment period.
To enroll in the Health
Enhancement Program (HEP) go to
follow the instructions. You will be asked for your
last name and employee number or the last 4 digits of your
social security number. You will be asked to either enroll or
decline enrollment in the plan. A failure to enroll will be
considered declining enrollment and you will be charged the
additional costs noted above. When you enroll you are verifying
that you agree to the required screenings and expectations of
HEP. There is also a link to the SEBAC agreement that provides
additional information on what those screenings and expectations
Once you complete the process you will be
asked if you want to print an enrollment confirmation for your
records. We highly recommend that you elect to print the
confirmation and retain it in the event there are any questions
regarding your HEP enrollment.

the HEP enrollment process, please contact Human Resources
immediately for assistance.
It is CRITICAL that you attend to this prior to September 15 if
you wish to avoid the increased costs associated with not
enrolling in HEP.
During open enrollment you are eligible to
change between plan providers and to add eligible dependents not
already covered on your plan. The Comptroller's Office
will not be sending Healthcare Planners as has occurred
in the past.
